  Your patient information forms are available on the next page of this website.  Please complete the Patient Intake Form, the Patient History Form, and any other forms your were asked to fill out and bring them with you to your initial appointment. Your thoughtfulness, thoroughness and honesty in completing these forms will assist us in providing you with the best care possible.

  Misc. Information

Please note: As a rule, most insurance companies consider nutritional evaluation and treatment to be “preventative” healthcare and will not reimburse for these services. Clearly, we do not share this view. However, we make every attempt when we prepare claim forms to separate charges for nutritional services from charges for conventional diagnostic services and chiropractic treatment.

Please do not work out in a gymnasium, do any vigorous activity, eat or drink anything (except water) 2 hours prior to you initial appointment. This will assure more accurate test results in your new patient workup and in-office lab tests. Also, you may be asked to change into shorts and a t-shirt so the doctor will have the ability to perform a thorough exam. You are welcome to bring your own shorts and shirt or we will provide them for you.

Cancellation Policy

We ask that you give us 48 hours notice if you need to reschedule this initial appointment. A 24 hour notice is required for rescheduling follow-up visits. We reserve the right to charge for missed appointments or those not cancelled 24 hours prior to the scheduled appointment.  These charges include consultation, examination and initial treatment.
